    We are currently in search of an experienced

    Population Health Care Director

    for our Baltimore, MD location.

    Population Health Management Director oversees operations and reporting of the Nurse Care Manager team,which includes Marylands Health Home and Medicares Value in Treatment case management programs at multiple clinics.

    With the aid of appointed team members, this pivotal member of the companys leadership team requires a dynamic professional who can provide oversight and direction to ensure the delivery of high-quality integrated healthcare services to our clients.

    The Director is responsible for strategic planning, growth, comprehensive administration and oversight of the Nurse Care Management team. This includes supervision of off-site and office-based staff, recruiting and retention, development, and implementation of program policies and procedures.

    In this leadership role, the Director will play a pivotal role in ensuring the delivery of high-quality integrated healthcare services to our clients while fostering a collaborative and supportive working environment for the program's staff.

    The Director will also be in charge of implementing new Health Home programs at the Concerted Care Groups planned additional treatment sites.


    Responsibilities:
    Collaborate with executive leadership to develop and implement strategic initiatives for the Population Health Nurse Care Manager ProgramAssess the evolving healthcare landscape and recommend adjustments to the program to meet changing needsMonitors Population Health performance at the population level and implements improvement effortProgram ManagementProvide overall leadership and direction to the Nurse Care Manager teamMonitor and evaluate program performance, ensuring compliance with regulatory requirements and organizational standards, and effectiveness of client careImplement quality improvement endeavors to enhance program effectivenessSpearhead and implement program initiatives, such as health fairs, targeted client education, and other periodic events to enhance client care and promote program growth and awarenessTeam Leadership and DevelopmentRecruit, train, and supervise Nurse Care Manager staffFoster a positive and inclusive work environment, promoting teamwork, communication, and professional growthDevelop, review, and implement program policies and proceduresStay abreast of changes in regulations and adjust policies accordinglyCollaboration and NetworkingBuild and maintain effective partnerships with external stakeholders, including healthcare providers, community organizations, and governmental agenciesDevelop and implement strategies to enhance the program's reach and impact in the communityFinancial OversightDevelop and manage the program budget, ensuring fiscal responsibility and efficiencyData Analysis and ReportingOversee the collection and analysis of program data, utilizing insights to make informed decisions and drive continuous improvementPrepare regular reports for internal and external stakeholders, highlighting program outcomes and achievementsDirect Patient CareWill provide direct patient care as needed in the same capacity as Nurse Care Manager staffQualifications:

    Master's degree in a relevant field (e.g., healthcare administration, public health, social work)

    Current Maryland State Registered Nurse License in good standing or valid Compact State LicenseProven experience in healthcare program management, with a focus on integrated care modelsStrong leadership and team management skillsKnowledge of relevant regulatory requirements and accreditation standardsExcellent communication, interpersonal, and problem-solving skillsDemonstrated success in driving program growth and innovationAbility to analyze and conceptualize data to ensure achievement of desired outcomesTechnically proficient in Microsoft Office programs and database useExperience working in Behavioral Health and Substance Abuse treatment preferredMUST be willing to travel up to 60% to our other sites (Hagerstown, Frederick, Cumberland, Baltimore, Fredericksburg, VA)We OfferCompetitive CompensationFull Benefits:
