    Job Summary

    This position requires a registered nurse with experience in home health care and case management. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.

    Responsibilities

    • Provide comprehensive assessments and develop individualized care plans to meet patient needs.
    • Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions.
    • Deliver high-quality nursing services and interventions to promote patient health and well-being.
    • Maintain accurate and timely documentation of patient interactions and care provided.

    Requirements

    • Graduate of an accredited school of nursing.
    • Current Washington State RN licensure.
    • National Provider BLS certification within 30 days of hire.
    • National Provider Identifier (NPI) number obtained within 30 days of hire.
    • 1 year of full-time experience practicing as a Registered Nurse.

    Preferred Qualifications

    • Bachelor's degree in Nursing or related field.
    • 1 year of recent clinical experience in Medicare-certified Homecare or Hospice agency/facility.

    Salary and Benefits

    We offer a competitive salary range of $85,000 - $110,000 per year, based on experience. Our comprehensive benefits package includes medical, dental, vision, retirement savings plan, life insurance, disability insurance, time off benefits, and more.
