    Description

    Director of Social Work - Nursing Home

    A key leadership position in our nursing home facility, overseeing the development and implementation of a comprehensive social services program.

    Responsibilities:

    • Program Development: Designs and directs a multidisciplinary social services program to meet the diverse needs of residents and families.
    • Census Management: Oversees all admissions and discharges processes to ensure optimal facility utilization.
    • Interdisciplinary Collaboration: Participates in a team-based approach with healthcare professionals to deliver high-quality care and services.

    Requirements:

    • Education: Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW)
    • Licensure: Current state-issued license as a Licensed Clinical Social Worker in Connecticut
    • Experience: Several years of direct social work experience in skilled nursing facilities, preferably with supervisory roles
    • Skills: Strong organizational and case management abilities, excellent communication and interpersonal skills, and expertise in long-term care
