Description

Care Planning and Scheduling Coordinator

In this role, you will be responsible for coordinating care services for patients and ensuring that they receive high-quality care.

Responsibilities:

Develop and implement care plans that meet the needs of patients.
Collaborate with healthcare professionals to ensure seamless coordination of care services.
Provide support to Case Managers and other staff members as needed.

You will have the opportunity to work in a fast-paced and dynamic environment, where no two days are ever the same.

We Are Looking For:

A highly motivated and organized individual who can think critically and solve problems efficiently. You will have previous experience with Home Health Scheduling using Wellsky and be familiar with the demands of this type of work.

Requirements:

Previous experience with Home Health Scheduling using Wellsky.
Candidates with 6 month to 1 year Home Health experience will be preferred.
Must possess and maintain good physical stamina and mental health.

Working Hours: Monday-Thursday 9:00am-5:00pm and Friday 9:00am-4:00pm.
