    Position Summary: Reporting to the Senior Director of Quality and Practice Transformation, the Process Improvement Specialist is responsible for management of the Electronic Medical Record (EMR) database, compilation and presentation of reports, and analysis of data related to all aspects of the Patient Centered Medical Home model and all other accreditation efforts to enhance the efficiency and promote transformation across all clinic processes and workflows.

    Duties and Responsibilities

    Process Improvement

    1. Facilitate and support ongoing performance improvement activities and initiatives to accomplish measurable project outcomes.
      • Use performance improvement methodology, tools, and techniques in organizational operations to accomplish objectives and outcomes that improve the effectiveness and quality of healthcare.
      • Work with organization leaders and managers in the planning, designing, implementation, and accomplishment of desired outcomes of key improvement projects.
      • Provide support and consultation to quality and operational improvement teams related to transformation and performance improvement initiatives.
      • Provide education on performance improvement methodologies to various PrimeCare teams and staff.
    2. Utilize Patient Centered Medical Home (PCMH) model and other accreditation models to analyze outcomes and work with staff to ensure compliance across all operational and clinical measures.
    3. Generate, monitor, and analyze data related to performance improvement for all staff.
    4. Work with organization leaders and managers in the planning, designing, implementation, and accomplishment of desired outcomes of key improvement projects.

    Data Analytics

    1. Build, run, and monitor reports in the electronic medical record. These reports provide data utilized for grant reporting, clinical workflows and effectiveness, phone utilization metrics and access to care, and process improvement coaching.
    2. Analyze and monitor reporting tools external to the organization, including, but not limited to, the phone system, lab vendor, and patient experience.
    3. Work collaboratively with organizational leadership to design metrics and tools for measuring process and team outcomes.
    4. Perform security and clinical audits and investigations using reporting functions within the EMR.

    EMR Subject Matter Expert and Administrator

    1. Serve as EMR subject matter expert for PrimeCare -- internally for staff, externally with third party vendors such as phone system and care management, and partnering with organizations such as Ascension.
    2. Provide technical support for the EMR and telehealth platforms. Coordinate with HR and managers on all account management during onboarding and offboarding of staff.
    3. Collaborate in overall design, coordination, introduction, and facilitation of the EMR platform, and management of projects and actions to improve patient care processes and outcomes.
    4. Chair the EMR Superuser Committee. Bring the recommendations from the Superuser Committee to Senior Leadership for EMR upgrades and enhancements approval.
    5. Serve as a resource for various PrimeCare programs to accelerate the discovery of new knowledge concerning efficiency, effectiveness of health care delivery innovations, and compliance.
    6. Other duties and responsibilities, as assigned.
    Required Skills or Abilities
    1. Strong verbal, written, and technical communication and presentation skills.
    2. Analytical and logical thinking skills.
    3. Working knowledge of electronic medical records (i.e., athenaone).
    4. Ability to work with spreadsheets and database software such as Microsoft Excel.
    5. Effective communication skills to properly understand the objective of the task and to be able to compile clear reports.
    6. Ability to collaborate and work in cross-functional teams as a contributor.
    7. Ability to multitask and work towards multiple projects at once.
    8. Efficiency in time management and ability to meet deadlines for multiple, simultaneous projects.
    9. Proven time management to lead and drive completion of projects, with limited supervision, to support business goals.
    10. Solid problem solving and strong attention to detail.
    11. Demonstrated proficiency in Microsoft office suite applications (Excel, Outlook, Word, and PowerPoint).
    Qualifications, Knowledge, Experience, or Licensure
    1. Bachelor's degree in healthcare, business, or public health. Master's degree and clinical experience preferred.
    2. 2+ years in ambulatory services environment or at an FQHC strongly preferred.
    3. 2+ years EMR experience preferred.
    4. Training in improvement methodologies such as Lean, Six Sigma or PDSA cycles helpful

    Physical Demands

    1. Must be able to remain in a stationary position 50% of the time.
    2. Must be able to move around the clinic site 50% of the time.
    3. Constantly operates a computer, computer printer, copy machine, and telephone.
    4. Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
    5. Must be able to transport from one site to another.
    6. Must be able to cover other shifts, as necessary.
