    Position: MDS Coordinator
    Location: Citrus Heights Post Acute

    Key Responsibilities:
    • Conduct comprehensive and accurate assessments utilizing your strong knowledge of Medicare and the Patient-Driven Payment Model (PDPM).
    • Collaborate with interdisciplinary teams to develop personalized care plans that meet the unique needs of residents.
    • Stay current with evolving regulations and best practices in Medicare and MDS processes.
    • Foster a culture of continuous improvement and learning within the MDS team.
    • Actively participate in professional development opportunities to enhance your skills and knowledge.
    Qualifications:
    • Current LVN or RN licensure.
    • Proven experience as an MDS Coordinator with a deep understanding of Medicare and PDPM.
