    Job responsibilities:
    Work 40 hours per week with 8-hour shifts.
    Screen for social determinants of health and incorporate findings into care plans as applicable.
    Communicate changes in discharge needs to the care team promptly.
    Conduct psychosocial assessments and identify barriers impacting care transitions.
    Facilitate care coordination across providers to support continuity of services.
