- Incorporates ethical principles and advanced practice professional standards in the care of patients and families receiving palliative care, while advocating for patients' and families' rights and access to quality palliative care.
- Demonstrates knowledge, skills and attitudes that are consistent with the advanced Scope and Standards of Hospice and Palliative Nursing Practice and ANA Code of Ethics for Nurses.
- Promotes an interdisciplinary/health care team approach to providing advanced hospice and palliative care nursing to the patient, family, community, and environment related to the potential or actual disease process and treatments.
- Demonstrates and promotes culturally and spiritually competent care by respecting diversity, regarding culture, spirituality, gender, socioeconomic statue, sexual orientation consistent with the advanced Scope and Standards of Hospice and Palliative Nursing Practice and ANA Code of Ethics for Nurses.
- Demonstrates knowledge, skills, confidence, and the application of adult learning principles when providing palliative care education to patients/families, health care professionals and community.
- Utilizes research to identify, examine, validate, and evaluate current theories and palliative care practice with the goal of improved outcomes for palliative care patients, families and the community.
- Manages and negotiates the health care delivery system to enhance quality of life for patients and families experiencing life-limiting, progressive illness and facilitate the provision of comprehensive, coordinated care within and across health care settings.
- Communicates effectively with palliative care patients, families, and health care professionals by applying knowledge of communication theory and principles.
- Maintains a friendly yet professional demeanor when interacting with the patient, caregiver, and family, as well as, with members of the interdisciplinary team and office support staff.
- Respects the professional boundaries of communicating personal information with patient and caregiver.
- Maintains confidentiality of privileged information and supports the patient's rights to privacy.

POSITION DESCRIPTION
A Nurse Practitioner who, operating within the framework of the mission and
philosophy of Hospice, evaluates and manages patients and provides nursing care to patients. They
work with the Interdisciplinary Hospice Team including the Hospice Medical Director to gather, document,
and share clinical findings to determine continued hospice eligibility, physical status and function within the
scope of APRN practice. The Hospice APRN will perform the face-to-face encounters for hospice patients
no more than 30 days prior to the start of the 3rd benefit period (180 days recertification) and then again
every 60 days thereafter.
Job Type: PRN
Salary: $55.00+/hr DOE
