- Thursday to Monday
- 40 Hours a week for 5 days
- 8:00AM to 5:00PM
- Provide the initial all system nursing assessment prior to the provision care.
- Complete initial, on-going comprehensive assessments and chart observations of the patient's condition and of the patient's needs at each visit.
- Coordinate appropriate care, encompassing various healthcare personnel (such as Physician, Physical Therapists, Occupational Therapists, Home Health Aides and external providers) that are in patient's care plan.
- Effectively plans and organizes work assignment, manages and sets competitive priorities, and completes work assignments with a minimum of direct supervision and within established time frames.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
- RN License in the State of Nevada
- Updated Credentials
- CPR / BLS
- Negative TB Test Result
- Driver's License and Auto Insurance
- Flu Shot / Signed Declination Form
- Hep B / Signed Declination Form
- Covid-19 Vaccination / Signed Religious Exemption Form
- Immunization Records (MMR, Tdap, Varicella)
- Will be driving to patient's residences
- Must be able to lift 25 lbs
- Must be in good health and be able to fulfill the physical requirement
- Must be willing to drive all-over Nevada
- With Home Health Care experience preferred
