Job summary
The Patient Navigator guides patients through the healthcare process and communicates with patients and their families regarding their procedure preparation recovery and follow up as well as communicate with referring physicians.Essential job functions include navigating patients through diagnostic evaluation educating supporting and empowering patients to make informed treatment decisions remaining a support system throughout the patient's treatment collaborating with patients families and providers in decision-making offering medical guidance acting as a support group guiding patients caregivers throughout the treatment process providing consultation coordination throughout diagnosis treatment and follow-up communicating with patients to schedule procedures provide preparation instructions follows up with referring physicians on procedure status keeping open lines of communication uses clinical expertise makes recommendations identifying develops relationships departments involved care of patents educating them on patient navigator role facilitates interaction communication health care staff involved patent's procedure.
