    Overview

    The Licensed Practical Nurse provides direct patient care and coordination of care services to assigned clients as delegated by a Registered Nurse or Advanced Practice Nurse to work alongside our outreach teams. Their primary responsibilities will be medication monitoring, administration, and reconciliation for outreach clients to assist the team with ensuring medication is packed correctly and to reduce medication errors, as well as medical care coordination and health education. The Licensed Practical Nurse will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.

    The Licensed Practical Nurse will work with clients directly in the community (70%) engaging them in their home settings.

    Responsibilities

    • Collect clinical data, including client mental and physical health symptoms, to collaborate in assessment of the health status of clients and the development and implementation of nursing plans of care.

    • Provide direct clinical care to clients which includes administration of specified medications including long-acting injectable antipsychotics, obtaining vital signs, and performing point-of-care testing.

    • Assist in transporting clients in the community to medical appointments when necessary.

    • Provide follow up care coordination and support following client discharge from hospitalization or ER visit.

    • Provide medication monitoring for clients which include packing medication boxes, changes in bubble packs, and oversight of medication training including controlled medications.

    • Reconciliation of medication for clients which include checking medication changes with doctor appointments and follow-ups, review of discharge paperwork and current medication lists, and consultation with pharmacies and healthcare providers.

    • Participate in client education and counseling, client advocacy, and care coordination in collaboration with the client, Registered Nurse, and interprofessional team.

    • Support clients with skill building around IADL's and ADL's in home and community.

    • Document relevant findings and interventions in electronic health record.

    • Communicate to appropriate supervisors and to client's psychiatric, primary care, or other providers as directed.

    • Attend team meetings to provide updates on client's current needs and department trainings.

    • Attend agency wide nursing meetings and trainings as scheduled.

    • Perform other related duties and/or projects that may be assigned to support the agency as a whole.

    Qualifications
    • High school diploma or GED
    • Licensed Practical Nurse under the Illinois Nurse Practice Act
    • Current active Licensed Practical Nurse in the State of Illinois through the Illinois Department of Financial and Professional Regulation
    • Two year's experience working with people with mental health and/or substance use disorders and medical comorbidities preferred.
    • Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
