    Physician / Internal Medicine Opportunity

    We are seeking a dedicated and compassionate internal medicine/geriatric physician to join our interdisciplinary team at The Eddy Senior Care, a member of St. Peter's Health Partners.

    Program Overview

    The Eddy Senior Care is an established PACE program that provides comprehensive care to frail seniors in the community, avoiding nursing home placement through relationship-driven support.

    • Care delivered in multiple settings: home, medical office, or selected nursing homes as needed
    • Focused on delivering person-centered care through relationships rather than encounters

    This opportunity allows physicians to work collaboratively with an interdisciplinary team while following participants across various environments.
