Licensed Practical Nurse (LPN) - Home Care - Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment.

Hours:
Full-time | Days 8:00am - 5:00pm |

Hourly Range:
$30.88 per hour plus weekend differential while working on Saturday and Sunday's.

• Territory: We are looking for someone to be able to travel throughout Interim's service area based on client's needs.

• Able to travel within geographic area serviced by the office. Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides services that are ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient and family in meeting healthcare related needs.
