- Graduated from an accredited LPN program
- Currently licensed/registered in the State where applicable. Must keep an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing during employment.
- Two (2) years of nursing experience. Experience in geriatric nursing is a plus.
- CRN C Certification (clinical compliance)
- CPR certification upon joining or obtain during orientation. Must keep CPR certification up to date throughout employment.
- Advanced knowledge in the field of practice
- Make independent decisions when necessary
- Familiar with practices, procedures, laws, regulations, and guidelines governing functions in the post-acute care facility
- Implement and interpret department programs, goals, objectives, policies, and procedures
- Perform effectively in all competency areas including patient rights and safety
- Maintain professional relationships with all associates, vendors, etc.
- Keep all proprietary and confidential information confidential
- Understand and follow company policies including harassment and compliance procedures
- Demonstrate integrity and professionalism by following Life Care's Code of Conduct and completing mandatory Code of Conduct and other relevant compliance training
- Complete and submit precise and timely MDS Assessments, CCAs, and Care Plans following CMS RAI Manual Regulations
- Notify the DON of any changes in a patient's condition identified during the MDS Assessment
- Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations affecting documentation
- Assist in reviewing the Interdisciplinary Comprehensive Care Plan
- Verify Final Validation Reports and ensure all assessments are accepted into the CMS QIES system before billing, and inform the Business Office if assessments are not ready for billing
- Check CMS Reports to identify late completed or submitted assessments and establish systems and processes to avoid recurrence
- Attend and contribute to the Daily PPS Meeting, Monthly Triple Check, and other meetings as needed in the absence of the RN MDS Coordinator
- Carry out staff nurse duties when required
- Provide excellent customer service and maintain a positive attitude towards patients
- Assist in patient evacuations
- Maintain consistent, punctual attendance
- Apply concentration, reasoning skills, and good judgment
- Interact and function effectively as part of an interdisciplinary team
- Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during work hours
- Read, write, speak, and comprehend the English language
