    Catholic Charities of the Archdiocese of Newark is one of New Jersey's largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties.

    We are currently seeking a Full Time Case Manager for our Supportive Services for Veteran's Families Program (SSVF), located in Jersey City, NJ. The SSVF program provides case management and temporary financial assistance to veterans and their families who are homeless or at risk of homelessness.

    The Case Manager is responsible for the development and management of a service plan for Veteran clients who are homeless or about to be homeless. This includes identifying needs and matching needs to appropriate services, along with arranging, coordinating case management services for individualized service plans, assuring appropriate quality services and conducting a timely intake and comprehensive assessment of all clients. The Case Manager will maintain a constant and consistent relationship with clients in accordance with program requirements to ensure they are reaching self-sufficiency. The Case Manager will also ensure that all client data is entered into the Homeless Management Information System (HMIS) Database in a timely fashion.

    • Bachelor's Degree in a related field
    • 1-2 years of related experience
