This role offers the opportunity to work in both our Memory Care and Traditional neighborhoods. Employees are fully cross-trained to provide consistent, resident-centered care throughout the community.

- Schedule: Full-Time, Part-Time, and PRN Shifts available
- Shift Times: 7:00AM-3:00PM, 3:00PM-11:00PM, and 11:00PM-7:00AM
- Sign-On Bonus: $1,000 sign-on bonus (part-time and full-time hires only)

- Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility
- Assist residents with self-administration of medications, including proper documentation
- Observe and report changes in residents' physical or emotional condition to nursing staff
- Provide supportive, respectful care that meets residents' physical, emotional, and psychosocial needs
- Communicate effectively with residents, families, and the care team to support coordinated care
- Follow care plans, infection control practices, and all safety policies and procedures
- Document care provided and participate in team meetings as required
- Active, Home Health Aide certification
- Proficient in reading, writing, and following instructions in English
- High school education or GED
- Demonstrate physical ability to stand, sit, walk, bend, squat, and lift/carry up to 20 pounds unassisted or 50 pounds with assistance
