    We're offering a great work environmentThe Preserve Campus is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.


    Responsibilities include:
    To organize and direct social services functions through direct service with the resident and family throughout pre-admission and admission processes, resident adjustment, ongoing and discharge services

    QUALIFICATIONS:
    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.     Bachelor's degree in social work or related degree in social sciences; licensed where applicable by the state; state requirements may dictate supervision and degree type.2.

    Minimum of one year of social work experience, preferably in a long-term healthcare setting; knowledge of long-term healthcare and public health settings; familiarity with Federal and State regulations and requirements for long-term care.3.

    Ability to read, speak and write fluently in English; excellent communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.4.

    Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals; ability to comprehend and carry out simple statistical functions to create standard graphs and tables.5.

    Ability and willingness to work flexible hours, such as during evenings and weekends, for planned activities and special events, family meetings and conferences, and as necessary, to facilitate admission calls, admissions and tours.

    Manages "on-call" schedule with other social work staff by pre-arrangement to cover facility for weekends/evenings admissions per Executive Director.6.

    Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write contracts and complex written arguments.7.

    Ability to work with residents of varying levels of responsiveness.8. Familiarity with word processing, data processing and computer entry skills to match existing clinical management software at facility.9. Ability to perform several job functions at once.10.

    Numerical ability necessary to make various computations in planning departmental objectives, keeping work records and schedules, preparing budgets, requisitioning supplies, and monitoring inventory control.11.

    Ability to apply principles of personnel administration to interviewing, selection and supervision of employees, and in monitoring department activities.12. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.


    ESSENTIAL FUNCTIONS:

    Develops a mission and direction for the department itself, participates in goal setting and direction activities with the Executive Director.b.

    Attends and participates in management committee meetings to represent department.2. Manages and directs activities of department in accordance with Volunteers of America National Services' policies and procedures and guidelines.a. Uses Volunteers of America National Services policy manual.b. Uses Volunteers of America National Services forms and processes.c. Seeks guidance of Executive Director in policy decisions.3. Coordinates resident admissions.a. Takes pre-admission calls and manages a waiting list as necessary.b. Offers tours to prospective customers and admissions.c.

    Conducts pre-admission screening (1) per phone; (2) in hospital or at referring Long-Term Care Facility; (3) at prospective resident's home.d.

    Functions as Admissions Coordinator as defined in the Admissions Procedures Manual.e. Plan and initiate hospital or in-home visits with current or prospective residents, as necessary.f. Provide and/or arrange for meeting with all new residents.

    Identify and provide for the medically-related social, emotional, spiritual and psychological needs; assist with resident orientation and adjustment to environment.g.

    Completes admission documents.h. Advises residents and families of advance directive policies.i.

    Assist with prompt referral to an appropriate agency or government program if financial assistance is indicated; assist with re-certification papers to maintain status of government programs such as Medical Assistance, Social Security, and S.S.I.; notify Business Office.4.


    Coordinates and leads admission transfer and discharge team:
    a. Prepares cases for review of Executive Director, Director of Nursing and team.b. Coordinates activities with Medicare meetings, etc.5. Completes assessments of residents as necessary for care planning, MDS, RAPS, PPS and other required documents.6. Initiate and/or participate in multidisciplinary care conferences held for residents, as appropriate to state and federal guidelines.a. Serves as resident advocate - coordinates invitation of residents and families.b. Refers to Volunteers of America National Services' MDS and care planning manuals to complete required sections.c. Implements described case management, counseling or support to resident as described in care plan.d. Maintains records as described by Volunteers of America National Services policy - maintains confidential records as necessary.e.

    Provide support services and conduct therapeutic group work for residents and/or families dealing with the aging process, guilt of nursing home placement, working with angry and despondent residents and related problems, when appropriate.f.

    Conduct and/or arrange for casework as needed; follow through on referred problems or psychosocial needs of residents. Maintain confidential department records on all casework.7. Initiates reporting and documentation in the medical record.a. Maintains accurate and appropriate social service documentation in the medical record.b. Updates assessments and documentation of resident transfers within the facility and/or to an alternate setting.c. Develop or ensure comprehensive social histories on all new admissions; update at regular intervals in accordance with facility policies.d. Prepare written summaries of family conferences and other related conferences.e. Attend initial care and quarterly review conferences; participate in problem identification, resident goal setting and approach planning.f. Ensure proper notification and documentation of resident transfers within the facility and/or to an alternate setting.g. Assume responsibility for proper documentation of resident discharge planning.h. Notifies Business Office of payer status changes.8. Serve as facility Discharge Planning Coordinator.a. Arranges for discharge planning meetings as necessary.b. Documents discharge plans.c. Maintains list of agencies and public service agencies that can assist residents, families with their discharge plans.9.

    Serve as an advocate for residents, upon request of the residents, by attendance and participation in Resident Council meetings; promote and adhere to Residents Rights Act.10.

    Advise and recommend solutions to the Executive Director and Director of Nursing of major problems or concerns that residents or their family members are experiencing.11.

    Advise and assist Nursing and Recreational Departments in ensuring residents have the necessary and adequate clothing needs, health aids, etc.12.

    Maintain supply of admission packets, application materials and necessary department forms.13. Maintain monthly Social Service referral/inquiry report.14.

    Develop and promote positive public relations and community resources through contacts with social service personnel in area hospitals, welfare departments and the Social Security Administration.a.

    Maintain a current Community Resource File.b. Visit referral agencies and local hospitals to share information on facility.c.

    Provide community resource information to residents and families relating to alternative health care services or facilities, community health education clinics, etc.d.

    Plan and present sessions to civic groups, youth groups, schools and church groups on problems of aging and adjusting to nursing home life.15.

    Administer personnel functions of Social Services Department, when appropriate.a.

    Interview and hire department personnel; supervise orientation; counsel and/or discipline employees on infractions of established rules; terminate employees when needed.b.

    Conduct performance evaluationsc. Develop, implement and maintain work routines and special events staffing schedules. Maintain an established staffing schedule, not exceeding budgeted hours per pay period for the department.d.

    Initiate and direct in-service training programs, such as annual all-staff in-service on Residents Rights and other associated legislative acts; maintain documentation.e.

    Schedule and present periodic department meetings.f. Advise Executive Director of department staffing needs.g. Review timecards verifying hours worked and authorize overtime.h. Instruct staff on various federal, state and facility's rules, regulations, policies and procedures; monitor compliance.i.

    Attend department head and all-staff meetings and in-service programs as scheduled; represent Social Services Department on various committees as requested by the Executive Director.j.

    Participate in approved ongoing educational programs.k. Participate in Executive Director-approved ongoing educational programs.16. Participate in department budgeting and planning processes.a.

    Confer with the Executive Director, on an annual basis, in the development and implementation of department objectives with respect to overall facility and organizational goals; review quarterly.b.

    Support and implement administrative policies established by the governing authority and/or by the Executive Director.c. Submit a brief monthly report, oral or written, to Executive Director concerning department functions, activities, and expenditures.d. Monitor in conjunction with the Executive Director, monthly department expenditures to budget.e. Collaborate with the Executive Director in reviewing annually department policies and procedures; make written recommendations.17.

    Serves on committees as required by Executive Director including, but not limited to:
    a. CQI committee;b. Safety committee;c. Management committee;d. Admissions committee;e. Marketing committee.18.

    Attends resident and family council meetings:
    a. Serves as liaison to Executive Director and management team.b. Facilitates groups as necessary.c. With permission, posts minutes.19.

    Serves as rights advocate liaison to the facility:
    a. Coordinates posting of all Federal and State required material for resident and family advocacy.b. Arranges and updates material as necessary.20.

    Advises and recommends solutions to the Executive Director and Director of Nursing of major problems or concerns that residents or their family members are experiencing.21.

    Ensures residents have necessary and adequate clothing, health aids, etc.22. Develops statistics for Executive Director on operation of department including admission/discharge/ referral/inquiry statistics.23.

    Acts as the facility Resident Protection Coordinator, coordinating investigations of incidents of potential abuse/neglect at the direction of Executive Director.

    Works directly with DON, HR and department managers to complete thorough investigation following and utilizing VOA policies and forms.24. Serves as the Grievance Official.25. Attends required in-services and completes assigned online modules.26. Performs other duties or special assignments as directed by the Executive Director.
