    Sunbelt Staffing, a nationwide healthcare staffing agency, is seeking a compassionate and experienced Hospice Home Health Registered Nurse (RN) for a 13-week contract position inLa Grande, OR.The successful candidate will join our client, a leading nationwide home health agency, and provide quality patient care in a home-based setting.

    Contract Details:

    • Location:La Grande, OR
    • Company: Sunbelt Staffing
    • Contract Length: 13 Weeks (with possible extension upon successful start and completion)
    • Hours: Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
    • Compensation: Negotiable, based on experience and territory budget
    • Mileage Reimbursement: 0.48c per mile from patient home to home
    • Blended Pay Packages: Available for travelers
    • Benefits: Medical, dental, vision, 401k match after 90 days

    Job Responsibilities

    • Conduct comprehensive patient assessments, focusing on comfort, pain management, and hospice care standards.
    • Develop and implement individualized care plans in collaboration with physicians, focusing on symptom control and quality of life.
    • Administer medications, monitor vital signs, and provide skilled nursing care, including wound care and other treatments.
    • Offer emotional support and guidance to patients and families, explaining the hospice process and addressing concerns.
    • Maintain accurate patient records in Homecare Homebase (HCHB), ensuring compliance with regulatory requirements.
    • Travel to patient homes within the La Grande, OR area using reliable transportation.

    Requirements

    • Active Registered Nurse (RN) license in the state of Oregon.
    • Minimum of 1 year of home health experience.
    • Minimum of 6 months of hospice experience preferred.
    • Proficiency in OASIS and Homecare Homebase (HCHB).
    • Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
    • Reliable transportation with valid driver's license.
    • Ability to meet Joint Commission onboarding standards upon offer.
