    This Register Nurse position is fulltime on site position located at Worcester Health Care for the Homeless (WHCH) (formally HOAP), which provides trauma-informed medical and behavior health services to individuals experiencing homelessness.

    The goal is to create a welcoming, empowering, and dignified medical home for those we serve.

    Our team-based approach provides care in our newly renovated clinic located at 199 Chandler Street as well as various outreach clinics within the City of Worcester.

    In addition to medical and behavior health services, patients can access case management, recovery coaches, and Office Based Assisted Treatment (OBAT) for substance-use disorder.


    Job Description:

    The RN Staff Nurse for Primary Care provides direct and indirect nursing care in a family practice setting. The RN will participate in all aspects of
    care to patients including triage, health maintenance teaching, and administration of vaccines/medication.

    The RN will also collaborate and communicate with other nursing professionals, medical assistants, and medical providers to ensure high quality patient care.

    The RN will provide support to medical providers, as needed in the management of prescriptions and controlled substance prescriptions.


    Educational Requirements:


    Graduate of accredited school of nursing required; AS or BS in nursing preferred.


    Experience Required:


    Two years full time professional experience or part time equivalent


    Professional Licensure/

    Special Skills And Certification Required:


    Must be licensed as a RN by the Massachusetts State Board of Registration. BLS required.

    population preferred, but not required.

    • Awareness of resources available within the health center to accommodate the needs of
    patients.

    care.


    • Processes personal health information in accordance with FHCW HIPPA policies and
    procedures.


    Supervisory Responsibilities:


    Precept medical assistant staff by assisting and providing direction with procedures and activities that facilitate patient centered care.


    Reporting Relationship:


    Reports directly to the Lead Nurse
