    Summary

    The VA Northeast Ohio Healthcare System is recruiting for an Registered Nurse. The Registered Nurse will function in Community Outpatient Service.

    Duties

  • Participates in a multi-discipline team approach to provide chronic disease management to a panel of primary care patients.
  • Provides telephonic care support to patients to include triage of acute care needs, care coordination, and care support.
  • Provides comprehensive assessment and care planning for individuals presenting with acute needs.
  • Supports the team in providing disease specific education to veterans and their families.
  • Demonstrates outstanding customer service to both internal and external customers.
  • Demonstrates outstanding collaboration with internal and external care providers.
  • Observes professional practice standards in the delivery of care.
  • Supports Medical Center initiatives such as HRO, Relationship Based Care, and Pathways.
  • Demonstrates and supports evidence based solutions to care issues.
  • Utilizes electronic tools to support care to include secure messaging, VVC, telephonic, and others.
  • Utilizes electronic data bases to identify high risk individuals and those with poorly controlled chronic disease issues.
  • Participates in team huddles, group meetings, and TEAMS chats as required.
  • Demonstrates understanding of delegation and responsibilities thereof.
  • Demonstrates professional nursing leadership in the clinical setting..

    Work Schedule: Monday - Friday; 8am - 4:30pm or 10am - 6:30pm (Hours are based on the needs of the Canton VA Outpatient Clinic)
