CASE MANAGER, CRISIS REACH CHILD AND ADOLESCENT - CHARLOTTESVILLE (FULL TIME)REQ#:
GENERAL STATEMENT OF RESPONSIBILITIES:
The incumbent serves as a Region 1 Regional Education Assessment Crisis Services Habilitation (REACH) Case Manager for adults and children with developmental disabilities and behavioral health needs in addition to those presenting with challenging behaviors (DD/BH) residing in Developmental Services Region I of Virginia which covers the nine Community Services Boards (CSBs) within Region 1.
This position is responsible for crisis response requiring de-escalation, clinical and coordination services, assessments, developing and implementing Crisis Education Prevention Plans (CEPP), linkage to other services, follow-up, and crisis stabilization services.
Position is a full-time (M-F), benefitted position with a shared responsibility as Primary and Secondary On-Call Clinician which provides a rotating 24/7/365 crisis line, including on-site response.
Applicants must live in the catchment area for which they are applying. Multiple openings, shifts and locations needed. Various Locations including Staunton, Culpeper, Lynchburg, and Fredericksburg.QUALIFICATIONS:
Qualified Mental Health Professional (QMHP or QMHP-T)– minimum of Bachelor's degree in human services or related field from an accredited institution and no less than one year of full-time relevant experience.
In addition, the incumbent needs to possess the following knowledge, skills and abilities:
Knowledge of:
a. Characteristics and service needs of persons with intellectual and developmental disabilities, mental illness, behavioral disorders, language disorders, and other sensory impairments;
b.
Behavior modification; Cognitive Behavioral Therapy (CBT); assessment and intervention strategies; emergency interventions; family, group, and individual therapies; biopsychosocial education; risk assessments; sex offender treatment; substance abuse interventions; etc.
c. Community mental health and public health in order to provide advocacy and enhance individual's treatment and access community resources;d. Principles, procedures, techniques, trends, and literature related to mental health, intellectual disabilities, DD, and substance abuse;
e. Psychiatric/medical medications/treatments;
f. Assessment and intervention strategies with persons with severe, multiple disabilities (counseling, training, etc.);
g. Non-aversive behavioral management techniques/crisis intervention skills;
h. Data collection capabilities;
i. Specific local, state and federal regulations;
j. Philosophies of normalization/ integration
Skills in:
a. Consumer needs assessment, service planning, development of habilitative programs and evaluation of individual progress;
b. General knowledge of counseling and behavioral management skills with dually-diagnosed persons;
c. General knowledge in the management of aggressive behaviors;
d. Organizing records;
e. Service delivery;
f. Negotiating services with contractual and other agencies;
g. Managing medical emergencies;
Abilities to:
a. Adhere to established policies and procedures;
b. Analyze situations accurately and develop clinically appropriate responses and/or interventions;
b. Communicate effectively as part of an interdisciplinary team in relaying information to facilitate treatment;
c. Perform clinical interviews in order to obtain information and to formulate appropriate treatment plans;
d. Develop reports, create presentations, and communicate findings to both internal and external audiences;
e. Utilize database systems and software to efficiently and effectively meet programming requirements and needs;
f. Flexibility in working modified schedules as required;
g. Willingness to drive and must possess a valid driver's license;
h. Develop and maintain professional and therapeutic relationships;
i. Promote positive, collaborative, and professional working relations among co-workers and representatives in the community;
j. Record data and complete documentation within established expectations and by communicated deadlines;
k. Work effectively to coordinate plans and assist in delivering quality services;
l. Assess physical environments in meeting client needs;
m. Respond effectively to changes in the work environment in a positive and professional manner;
n. Work in and contribute to a collaborative team environment.
