- Respond promptly to after-hours calls, providing skilled nursing care and emotional support to patients and families facing end-of-life issues.
- Current Registered Nurse (RN) licensure (in good standing) in Pennsylvania with one year of experience under license
- Oversee the implementation and ongoing assessment of the patient's plan of care
- Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
- Works well with patients on a 1-1 level with strong patient care team support.
- Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.
- Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
- Admissions and On-call experience preferred but will train.
- Must be willing to drive with reliable transportation, a valid driver's license, and auto insurance, to be able to travel across the service area.

Care Comfort Hospice -REGISTERED NURSE WEEKEND ON CALL
As an On-Call RN, you will work outside of regular hours, to patients where they reside, including private homes, assisted living facilities and Skilled Nursing Facilites. This position will be after-hours on-call. Hours will consist of: Friday 5 PM to Monday 830 AM. You may sleep during your shift, however you MUST wake up to incoming phone calls to address patient needs.
This role will consist of answering after hours calls, triaging patient needs, seeing declining patients, completing evening admissions, and faxing new orders.
We are growing and want to expand our Hospice Team to help provide the best care for our patients.
