    The Registered Nurse in Home Health will provide and coordinate nursing care for patients in their homes based on physician prescriptions and in compliance with regulations. The role involves collaborating with healthcare professionals, patients, families, and community resources to ensure seamless patient care within a Point of Care environment.

    • Deliver clinical services within the defined scope of practice and as per state nursing regulations
    • Perform thorough nursing assessments, develop care plans, and maintain accurate documentation
    • Regularly assess and update patient care plans based on changing needs
    • Ensure patients meet home health eligibility criteria as per payer guidelines
    • Collaborate with physicians and other professionals to modify and implement care plans

    License Requirements:

    • Active RN license in the state of practice
    • CPR certification
    • Valid Driver's License, vehicle insurance, and access to reliable transportation
