- Manage patient continuum of care by addressing care gaps and conducting timely follow-ups.
- Conduct wellness exams and physicals.
- Diagnose and manage chronic conditions such as hypertension, COPD, and diabetes.
- Address acute presentations including upper respiratory infections, colds, coughs, and flu.
- Facilitate appropriate referrals to specialists as needed.
