- Provide nursing care to patients and proactively support the efforts to ensure that standards for patient satisfaction, patient
- Work as a liaison between patients, staff and physicians and support the Advanced Care Model.
- Provide general nursing care to patients including vaccine administration and treatments in accordance with Licensed
- Review patient charts for missing labs, reports, and/or open orders and review as part of care team huddles.
- Verify chief complaint and take vital signs as requested. Review the EHR (electronic health record) active medication
- Conduct and document point-of-care tests based on protocol.
- Administer medications as directed by provider.
- Assist physicians with examinations, diagnostic procedures and treatments.
- Document all activities/interventions, patient/family responses, medication dispensed/ prescribed, etc., in the
- Manage in-basket requests and respond to all requests in a timely manner.
- Serve as a clinical liaison between the call center and medical office.
- Identify patients who may need point-of-care testing as well as patients with special needs (i.e., transportation)
- Adhere to infection control/safety guidelines, and confidentiality policies.
- Perform other job-related duties as required.
- Completion of a New York State Education Department approved Licensed Practical Nurse education program (R)
- Current New York State Licensed Practical Nurse License (R)
- Basic Cardiac Life Support certification (P)
- Minimum of one year Licensed Practical Nursing experience. (R)
- Knowledge of basic nursing principles and protocols. (R)
- Knowledge of policies and procedures related to infection control, environmental safety and patient confidentiality. (R)
- Skill in appropriate use of universal precautions, safe workplace and confidentiality methods. (R)
- Skill in health information management by appropriately charting patient data. (R)
- Ability to work effectively as team member with physicians and other staff. (R)
- Strong verbal and written communication skills. (R)
- Ability to appropriately interact with patients, families, staff and others. (R)
- Ability to flexibly respond to changing demands. (R)
- Experience working in medical office. (P)
