- Manage chronic conditions, treat acute illnesses, and respond to urgent medical needs and collaborate with physicians on treatment plans.
- Manage medication therapy effectively including dosage adjustments, discontinuing or changing meds as appropriate and avoiding high risk meds.
- Simplify medication regimen for improved adherence and safety.
- Order and monitor diagnostics including laboratory studies, radiological studies, etc.
- Conduct health screenings, administering immunizations, and performing routine physical exams to facilitate closure of potential care gaps.
- Document patient information accurately and communicate effectively with patients and healthcare team members.
- Recommend tools and devices that may improve the quality of life for the patient.
- Holds active, unencumbered Advanced Practice license in FL along with an Active DEA license.
- Working knowledge of computers and ability to document effectively and efficiently in an electronic medical record system.
- Bilingual preferred
