Clinician Adult Services Half time in our outpatient clinic providing individual group case management services to adults half time off site at contracted locations THE clinician will support clients access treatment which may include meeting in community homes shelters care coordination collaboration with program staff is key component of position engage youth services supports KEY RESPONSIBILITIES Conducts clinical assessments engagement determines DSM V diagnosis develops treatment plans implements modifies discharge planning provides case management completes documentation maintains productivity targets participates presents Peer Consultation Quality Assurance Reviews fosters collaboration identifies develops approaches address unmet needs service gaps assumes responsibility special tasks leadership projects contributes agency functioning represents Village community groups committees CULTURAL COMPETENCY Strong diagnostic assessment skills individual family group therapy experience culturally diverse populations variety clinical presentations knowledge behavioral health child welfare systems resources organizational communication skills ability function autonomously seek consultation appropriately word processing general computer skills preferred.
