- Make a positive impact on people's lives by conducting Adult/Geriatric Assessments, medical history review, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation according to Matrix guidelines and protocols
- Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and any relevant ancillary medical staff as needed
- Work closely with the Primary Care Physician (PCP) on patient education and provide follow-up care
- Utilize a nationwide network of advanced practice providers to establish and maintain relationships
- Services may be delivered in various settings including; Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree required OR equivalent experience and successful completion of NP licensure
- Master's Degree required OR equivalent experience and successful completion of PA licensure
- Current RN and NP/PA licensure in state of practice with prescription authority or the ability to acquire prescriptive authority
- Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty
- Hold a current BLS, ACLS, or CPR certification
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant. Willingness to travel within or outside the state
- Strong computer skills and familiarity with EMR software
- Demonstrate exceptional verbal and written communication skills with patients, clients, and colleagues
- Display a positive and sociable demeanor, and be a team player. Comfortable and adaptable to frequent changes
- Ability to travel, possess a valid state driver's license, and capable of driving a car with sufficient automobile insurance coverage for the state of residence

Responsibilities
About the Position:
During a visit lasting up to one hour, Matrix providers assess and observe a member's current health, medical history, medication adherence, social environment, and other risks. This provides valuable insight into a member's overall health and well-being that can be challenging to obtain during routine office visits. The Matrix Comprehensive Health Assessment aims to enhance the quality of care and potentially address multiple care gaps in a single visit.
Nurse Practitioner/Physician Assistant - As Needed – Home Risk Assessments
Type: As Needed-Flexible-Nurse Practitioner/Physician Assistant
Location: Home, SNF, Telehealth, and other community settings in Webster, New York
Hours: Days, Evenings, Nights, weekends.
PAY: $90.00 or $115.00 per completed Assessment based on county 25 hours or more/month-Flexible Scheduling
Benefits: As Needed providers do not receive benefits except for limited Voluntary Benefits
Mileage Reimbursement provided for all miles when 20 or more assessments are completed per month No show payment for 10 or more scheduled assessments per month
Matrix Provider – What to Expect:
THIS IS NOT A FULL TIME JOB – It is As Needed/Coverage with no guaranteed hours
