Registered Admissions Nurse

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients. They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families, and other caregivers. The ideal candidate should be detail-oriented, patient and calm. Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life's journey.

Duties and Responsibilities

• Carries primary responsibility for evaluation of all patients referred for hospice services.
• Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
• Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
• Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
• Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
