    Job Title: HOME CARE PHYSICAL THERAPIST (PT)

    About Us

    At Liberty Home Care, we understand the significance of recovery at home in improving patient outcomes following an illness, trauma, or surgery. Our dedicated healthcare professionals are committed to providing patients with independent recovery.

    Job Description:

    • Deliver patient care services alongside direct treatment and follow-up.
    • Evaluate new patients where physician orders exist and develop a tailored plan of care based on their diagnosis.
    • Maintain accurate, thorough, and timely documentation of patient care data, reflecting evidence of improvement, progress, or decline in the patient's condition.
    • Submit daily/weekly patient visit schedules to the Scheduler.
    • Foster necessary communication with internal and external contacts.
    • Discuss company and third-party reimbursement policies and guidelines with patients/families and collaborate with Patient Care Coordinators, physicians, and patients/families on treatment plans.
    • Collaborate with Patient Care Coordinators to refer patients to relevant disciplines.
    • Provide in-service education, healthcare professional education, and public education while supervising peers and supporting personnel as needed.
    • Conduct research and assist with new employee orientation as required.

    Requirements:

    • Bachelor's degree in physical therapy from an accredited institution and current licensure for the state where therapy will be provided.
    • Prior experience in non-home healthcare environment preferred, with a minimum of one year's therapeutic experience.
    • Strong knowledge of physical therapy theory and concepts, with the ability to evaluate and treat patients under a physician's order.
    • Minimum of twelve hours of continuing education annually.
    • Acceptance of assigned travel and working conditions.

    Estimated Salary: $70,000 - $90,000 per year
