    HOSPICE MEDICAL SOCIAL WORKER

    Full Time

    Job Description:

    • Deliver direct patient care services and accurately document patient care information.
    • Conduct clinical assessments to create personalized care plans and establish treatment goals for patients.
    • Utilize a solid understanding of psycho-social principles and family dynamics. Identify and evaluate social, emotional, financial, and environmental factors that could impact the medical treatment plan and expected outcome.
    • Collaborate with agency staff and liaise with other services to ensure optimal care provision.
    • Offer support during homecare visits as necessary.
    • Build strong relationships with patients and their families, responding sensitively to verbal and nonverbal cues.
    • Employ community resource planning to deliver patient services and educate patients and families on how to access these resources.
    • Assess the bereavement needs of families through initial checks and coordinate with the interdisciplinary team.
    • Provide grief care and bereavement support to patients and caregivers.

    Job Requirements:

    • Hold a Master's degree in Social Work from a Council on Social Work Education-accredited institution, with at least one year of hands-on experience, preferably in the home health/hospice sector.
    • Possess strong interpersonal abilities to communicate effectively within a multidisciplinary environment.
    • Be willing to accept assigned travel and working conditions.
    • Complete a minimum of twelve hours of continuing education annually.
