Temah Family Practice is seeking a Family Nurse Practitioner or Adult Nurse Practitioner for part-time or full-time work.

Key Responsibilities:

• Diagnose and treat acute illnesses, infections, and injuries related to medical conditions.
• Manage and treat common chronic conditions such as medical illnesses
• Prioritize telemedicine/telephonic consultations

The ideal candidate will have experience in chronic care management of medical illnesses, diagnosing and treating substance use disorder including opioid use disorder. Additionally they should be skilled in medication management including suboxone and monthly vivitrol injections. This role involves collaborating with physicians, mental health teams, and other healthcare professionals to ensure seamless patient care.
Responsibilities also include ordering diagnostic studies; managing medications; referring patients to specialists when necessary (including mental health); connecting patients with physical therapy resources;
Administering age-appropriate immunizations;
Advocating for patient needs
