    Summary:

    That's Good HR is seeking a skilled Case Manager to work with clients in Indianapolis and surrounding areas. As a trusted partner in employment placements, we specialize in handpicked positions for various industries.

    Description:

    Responsibilities:

    • Conduct thorough assessments to identify client needs, health conditions, and support systems.
    • Perform eligibility screenings to determine care criteria for public-funded programs.
    • Develop and maintain personalized care plans.
    • Maintain accurate and complete documentation for program requirements.
    • Build relationships with local organizations, community resources, and providers.

    Qualifications:

    • Availability to work 8am-4:30pm or 8:30-5pm schedule.
    • One of the following backgrounds is required:
    • Employment as a Case Manager by Area Agency on Aging (AAA) since June 2018, or a registered nurse, licensed practice nurse, or an Associate's degree in nursing with at least one year of experience serving older adults or disabled populations;
    • Bachelor's Degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services; or a Bachelor's Degree in any field with a minimum of two years full-time direct service experience with older adults or person with disabilities;
    • Master's degree in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services; or an Associate's degree in any field with a minimum of four years full-time direct service experience with older adults or person with disabilities.

    This position is based in the office until hired permanently, allowing for some hybrid flexibility. All candidates must reside in Indianapolis or surrounding suburbs.
