The Crisis Care Center Clinician will assess admit refer clients presenting to the Crisis Care Center provide direct clinical services such as individual group therapy initiate discharge plans.Primary Job Functions include overall planning coordination admissions working directly harmoniously with psychiatrist CRNPs SUD Assessor nursing staff conducting initial intake assessments substance use assessments telehealth services creating recovery-oriented treatment plans initiation discharge planning providing direct clinical services coordinating ongoing care emergency departments hospitals community health mental health providers documenting daily weekly reports treatment team meetings assigned duties tasks.
