Job summary
This position will drive to patients living in the following assigned WA state cities: Lynnwood, Edmonds, and Seattle. Responsible for assessing, planning, implementing, evaluating and providing skilled care to patients requiring physical therapy services in the home.Qualifications
- Current PT licensure to practice Physical Therapy in Washington State
- This position requires that you obtain an NPI number within 30 days of starting in the position.
- Recent applicable clinical experience in a medical/ surgical unit, rehab, or home health setting.
