Job Summary

Manages the care of a specific group of patients throughout the treatment process by collaborating with other healthcare providers and doctors. The social worker/case manager plays a key role in achieving high-quality, cost-effective results by providing the necessary support and utilizing resources effectively. Conducts psychosocial assessments and interventions to address emotional, environmental, and financial obstacles to optimal outcomes and quality of life.

Job Functions:

• Conducts psychosocial assessments and monitors patients' emotional well-being, offering crisis intervention and counseling to individuals and families to help them cope and adjust.
• Coordinates interdisciplinary treatment planning, ensuring patient input, monitoring progress, adjusting care plans, and ensuring the delivery of appropriate services.
• Facilitates communication between patients and all involved parties, organizes family meetings, and acts as a patient advocate.
• Handles insurance reviews, pre-certifications, and serves as a liaison with insurance providers.
• Plans and oversees patient discharges to achieve positive outcomes, including transfers to other healthcare facilities.
• Coordinates post-treatment care for reintegration into the community or workforce.
• Participates in support groups within designated areas.
• Maintains knowledge of financial and insurance-related matters and service costs.
• Contributes to educating patients and their families during treatment and follow-up care.
• Ensures efficient resource utilization based on patients' clinical progress.
• Demonstrates behaviors that promote a positive work atmosphere.
• Attends mandatory meetings and training sessions, adhering to hospital policies, procedures, and ethical standards.
• Documents information accurately in compliance with hospital and program guidelines.
• Meets program productivity benchmarks.
• Performs additional responsibilities as directed by the Clinical Leader.

Required Minimum Education:

4-year Bachelor's Degree

Specialty/Major: Bachelor's degree in social work, LSW

Preferred Education:

Specialty/Major: MSW and familiarity with medical rehabilitation preferred
