- PRN: As needed
- Per diem, paid per visit
- Visits the patient/family for an initial assessment of music therapy needs, when requested, in accordance with the interdisciplinary Plan of Care.
- Assists in determining and recommending appropriate music therapy experiences utilizing singing/instrument work, song writing, music relaxation, drum work, breath work and/or rhythmic movement.
- Evaluates the patient's needs and interests and establishes a treatment plan.
- Directs and participates in instrumental and vocal music activities designed to meet patients' physical or psychological needs.
- Assists the patient with life review through music.
- Assists the patient with increased socialization through music
- Assists the patient with music therapy to improve physical comfort.
- Helps the patient/family to understand, accept and follow music therapy recommendations and provides musical interventions planned to assist the patient in achieving optimum benefits.
- Documents all services provided to hospice patient and families.
- Records and reports the patient's response to treatment and any changes in the patient's condition.
- Completes all required documentation in a timely manner according to Good Shepherd Hospice policies and procedures.
- Maintains confidentiality of patient/family status.
- Completion of an accredited program in music therapy.
- Must be certified in the State in which they are providing Music Therapy.
- Demonstrated commitment to the hospice philosophy of care.
- At least one year of experience in a health care setting. Previous experience in
