    Job Summary:

    The Occupational Therapist in Home Health assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment, among other occupational therapy requirements defined by medical conditions.

    • Deliver services within the scope of practice as defined by state laws governing Occupational Therapy, adhering to the plan of care, using evidence-based techniques, and coordinating with the healthcare team.
    • Evaluate patient functional status, body system status (as required for CMS documentation), and occupational therapy needs, consulting with physicians to develop the therapy plan of care.
    • Monitor patient responses to treatment and report changes in condition to nurse supervisors and physicians.
    • Conduct patient assessments, applying diagnostic and prognostic functional ability tests to evaluate level of function.

    Requirements:

    • Current Occupational Therapy licensure in the state of practice
    • Current CPR certification
    • Valid driver's license, vehicle insurance, and reliable transportation or access to public transportation
