Roles and Responsibilities of a Home Health RN:

Provide home-based care to patients recovering from illnesses, injuries, and chronic diseases
Collaborate with physicians and supervise LPNs, CNAs, and aides
Conduct patient assessments, monitor progress, and report changes
Educate patients on care plans, medication, therapy, diet, and exercise
Guide family members on care plans, medication, and home safety
Ensure goals are achieved and assist in discharge planning

Requirements for Home Health Registered Nurses:

Graduate of an accredited nursing program with an active RN license in WI
Minimum of 1 year of nursing experience, preferably in home healthcare
CPR certification and knowledge of home health regulations
Clinical proficiency in nursing specialty, excellent communication skills, and compassion
