    Touchstone Health of San Antonio is actively seeking a qualified and experienced Home Health and Hospice Quality Assurance Nurse. The ideal candidate is an experienced home health and hospice nurse with a background in leadership & quality assurance to ensure that our agency is adhering to both state and federal guidelines for quality control.     Duties & Responsibilities:

    • Audits clinical documentation to ensure quality regulations and measures are being met.
    • Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accreditation standards and state and local laws.
    • Participates in administrative functions related to quality programs/functions as assigned.
    • Observes confidentiality and safeguards all patient related information.
    • Serves as a resource person to employees.
    • Assists in the investigation and reporting of any problem relating to patient care and/or policies and procedures.
    • Assists in the development of QAPI activities with appropriate data collection, aggregation, analysis, taking action and reporting of results according to Hospice's QAPI plan(s).
    • Conducts needs assessments using a variety of Strategic tools.
    • Delivers competence based programs for staff with a focus upon improvement of skills and retention of knowledge.
    • Prepares short and long-term planning and programs to address identified needs.

    Requirements

    Qualifications & Requirements:

    • Current and unencumbered license to practice as a Registered Nurse in the state of Texas
    • Minimum of three (3) years of hospice nursing experience
    • Experience in Quality Assurance and/or leadership preferred
    • Current CPR certification
    • Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years
    • Excellent written and verbal communication skills
    • MS Office skills (Word, Excel, PowerPoint)
