- Uses the nursing process in accordance with the standards and services throughout the continuum of care.
- Monitoring Veteran progress on an ongoing basis to evaluate care plans and revise discharge plan as indicated.
- Participates in the discharge plan of care for patients based on patient and community resources.
- Working with the IDT to provide a discharge care plan that is appropriate, safe and cost effective.
- Providing continuity of care by ensuring patient has supplies and equipment.
- Making home health referrals, initiate/coordinate new dialysis referrals for inpatients as needed.
- Initiate and coordinate fee basis home health services for complex inpatient needs receiving home health such as IV antibiotic administration, going home on a ventilator/tracheostomy/tube feeding. VA offers a comprehensive total rewards package:

Summary
The Discharge Case Manager is a Registered Nurse (RN) who is a member of the Nursing Service. Under the direction of an assigned preceptor or experienced staff RN, The RN-Discharge Case Manager I-1, uses the nursing process to work cohesively at managing all the Inpatient caseload of Veterans through coordination of care with the Interdisciplinary Team (IDT) members.Duties
Pay: Competitive salary, regular salary increases, potential for performance awards
