    Job Summary:


    The Community Health Center of Buffalo (CHCB) is seeking a full-time Physical Therapist to join our large, diverse, and well-established health center in Buffalo, NY.

    As a Physical Therapist, you will be responsible for developing and implementing physical therapy services for patients and working as part of a multidisciplinary team to provide quality healthcare.

    Responsibilities:

    • Evaluate patients' conditions and needs through assessments and review of medical history
    • Develop individualized physical therapy plans, including activities, goals, and desired outcomes
    • Demonstrate exercises and techniques, and ensure patients' understanding and ability to perform treatments
    • Work with and educate patients' families as needed
    • Make recommendations about special equipment and provide instructions on their usage
    • Assess and record patients' activities and progress in electronic medical records
    • Provide reports to other healthcare providers for coordination of care
    • Participate in Quality Improvement and Quality Assurance activities
    • Provide education to patients
    • Provide clinical direction to Physical Therapist Assistant and support staff
    • Adhere to clinical policies and procedures, including infection control and confidentiality
    • Assist with administrative triage when necessary
    • Comply with guidelines from regulatory agencies governing healthcare delivery
    • Develop and review policies and procedures
    • Attend meetings as required
    • Perform special assignments as requested
    • Direct, instruct, and evaluate medical students assigned to CHCB


    Qualifications:


    • Master's Degree in Physical Therapy
    • Possession of a license and current registration to practice Physical Therapy in New York State
    • Computer literacy and proficiency in MS Office
    • Experience using an Electronic Medical Record System
    Preferred

    Qualifications:


    • Doctorate Degree
    • Specialty focus certifications earned through the American Physical Therapy Association
    • Basic and Advanced Cardiac Life Support Certification
    • Professional Membership Affiliations
    • Experience with eClinical Works

    Physical Demands:
    This position requires periods of sitting, standing, walking, bending, and stooping. Significant job stress may be associated with the position, and evening work may be required.


    Salary:
    $68,000 - $85,000 depending on experience.
