ResponsibilitiesThis position is located in Durant, OK. It is a Full Time position that will support the Home Health Office.
The Home Health LVN/LPN Scheduler assumes the responsibility of coordination of care and staffing functions in accordance with state, federal and local regulations. Provides excellent communication to staff and manages workload to ensure job satisfaction. Exceeds expectations of patients and caregivers with exceptional communication and timely coordination of care. Exceeds practitioners expectations with professional and respectful communication when collaborating care. Triage and prioritize, based on clinical need, patients frequency, scheduling needs, and services. Obtains orders from practitioner's when needed to ensure approipriate care is delivered.
Texoma Medical Center, a -bed acute care facility has been providing quality health care to the residents of North Texas and Southern Oklahoma since 5. Our main campus is located in Denison, Texas, approximately one hour north of the Dallas/Fort Worth area and just south of the Texas/Oklahoma border. Additionally, nursing will provide professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times. The Standards of Practice set forth by the professional nursing associations will provide guidelines for the competent, safe and professional practice.
