- Demonstrate adherence to the PHP/SBHI Patient Experience philosophy, guidelines and tools.
- Demonstrate strong ethics, honesty, and adherence to PHP/SBHI Code of Conduct and PHP/SBHI Compliance Program.
- Read and understand route schedules and maps.
- Keep schedule in electronic record. Maintain vehicle logs.
- Provide transportation for patients to and from program services as appropriate and necessary.
- Familiarize self with the layout of the Dayton and Montgomery County, OH area. Read and understand route schedule to plan appropriate route each day.
- Maintain time schedule to arrive and depart with patients in a timely manner.
- Maintain certification in First Aid and CPR.
- Know how to handle emergency situations should they occur (i.e., aggressive, sick, injured, etc. patients).
- Ensure the safety of patients while entering, riding in, or exiting the van.
- Keep vehicles gas tanks filled.
- Inspect vehicles. Report need for servicing to the appropriate person. Initiate estimates for servicing. Obtain PO from SBHI Finance Dept. Once approved, arrange for servicing of vehicle.
- Ensure transportation schedule is entered in staff schedule within CareLogic.
- Attend monthly staff meetings or as requested by Program Director.
- Maintain current transportation caseload. Submit to Program Director monthly.
- Ensure monthly tracking of vehicle mileage. Forward to SBHI CFO, Program Director, and Chief of OP Clinical Operations monthly via email.
- Maintain transportation referrals. Forward to Program Director.
- Provide tracking of vehicle repair costs.
- Perform monthly fire extinguisher and lighting checks and periodic disaster drills. Process timely filing of safety records and documentation of SBHI safety drills.
- Inspect and correct safety hazards in the facility and parking area. Maintain safety equipment (i.e. spill kits, first aid kits, etc.)
- Demonstrate knowledge of all SBHI safety policies and procedures, including the SBHI Emergency Management Plan.
- Complete monthly inspections of all SBHI agency vehicles.
- High school graduate or equivalent.
- Valid Ohio Driver's license.
- Basic knowledge of health and safety issues and procedures governing health and safety.
