Job DescriptionOverview
Work Schedule: 8:00am to 4:30pm Monday to Friday. No weekends or holidays.
Coordinates patient care from diagnosis, through treatment, and after discharge. Assists in the coordination and planning of multiple healthcare services. Maintains communication with patients, survivors, families and health care providers to optimize access and monitor patient satisfaction.
Facilitates a seamless transition of care in collaboration with patients, physicians, vendors and all stakeholders involved in the care of the patient.
Serves as a liaison between referring providers, department providers and facility providers.
Health insurance experience is a plus Must be comfortable having difficult conversations with patients and be able to communicate professionally via phone or email.
