Overview
Dignity Health Yavapai Regional Medical Group is seeking a full-time outpatient Geriatric Nurse Practitioner to join our established and growing practice in Prescott, AZ.
Under direction from the CMO, the candidate provides patient care in an outpatient setting by diagnosing and treating disorders and diseases, performing patient exams, ordering or conducting diagnostic testing, and developing ongoing treatment and disease-management plans.
In this role, the candidate will also prescribe medication and coordinate referrals as necessary, conduct early-detection screenings and maintain accurate patient records and charts.
Effective communication with patients, physicians, staff, and administration is crucial.The ideal candidate:

This is a Dissemination site of UCLA ADC Program, started in the Summer of 2021. Patient population are those with dementia and their caregiver(s).
This position will be Alzheimer's and Dementia Care focus.
Project volume: 250 patients
Office Patients:
Patients are seen in the office with some off-site home visits in the surrounding area.
Typical work week (hours worked; hospital vs. clinic) - M-F 8hrs a day - Working in both the clinic setting, as well as, off site visits throughout the community to accommodate the patient's needs. Typical patient visit load of 2 new consults a day and f/u visits.

Current Board Certification
Valid medical license
Valid DEA certificate
2-5 years' in geriatrics. Focus on geriatric cognitive diseases preferred.
Pay Range
$52.79- $63.98/hour
