    What You Will Do :

    • Responsible for quarterly assessments, the MDS/Rap modules, and input into the care plan process.
    • Oversees all admissions to the facility for JCAHO standards. Ensures all paperwork is properly executed (i.e., Admission's Comprehensive Assessment, MD Orders, lab work, etc.).
    • Ensures weekly wound documentation is appropriately noted.
    • Evaluates, recommends, and assists with resident treatments.
    • Assumes responsibilities for providing all CNA's with their daily assignments.
    • Responsible for maintaining all resident profiles.
    What You Will Need :

    1 Ability to read, write and comprehend English and to follow oral and written instructions.


    • Interest in the needs of the aged and the chronically ill with the ability to work with the elderly.
    • Ability to work hours as scheduled based on the requirements of the position/assignments.
    • Registered Nurse licensed in the State of New York. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred.
    • CPR certified.
