    Description

    Job Title: Licensed Practical Nurse (LPN) at Community Based Outpatient Clinic (CBOC)

    The Licensed Practical Nurse (LPN) functions as a valuable member of the Teamlet within the Community Based Outpatient Clinic (CBOC) or as support for specialty clinic(s).


    The Teamlet comprises the Primary Care Provider, RN Care Manager, Clinical Associate, and clerk collaborating under the Patient Aligned Care Team (PACT) model.


    Qualifications:


    • Applicants must meet basic requirements including United States Citizenship, English Language Proficiency, and Physical Standards
    • Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse is mandatory
    • Education: Graduation from an approved school of practical or vocational nursing with additional criteria based on experience and training
    • Grade Determinations: Completion of specified years of qualifying experience with outlined competencies at the GS-6 level
    • References: VA Handbook 5005/27, Part II, Appendix G13 provides the QUALIFICATION STANDARD for Licensed Practical or Vocational Nurses
    • Physical Requirements: Satisfactory physical examination is required

    Duties:


    • The LPN in this role holds advanced responsibilities including monitoring workload credit, developing reporting procedures, and participating in performance improvement activities
    • Duties entail managing patient care deviations, assessing vital signs, triaging patient needs, and assuming a resource role for specialized procedures
    • Other responsibilities include maintaining knowledge of VA protocols, promptly recognizing urgent situations, and participating in specialty care clinic operations
