What We're Searching For

Hometown Home Health is seeking a dynamic individual to join our team as a speech language pathologist. Ideally you are a licensed Speech Language Pathologist in the state of Florida who prioritizes patient care and is able to commute within the Clay County area.

What You'll Do:

Provide Speech Pathology services and assessments to patients in their homes
Evaluate patients' level of functioning and recommended mechanisms to enhance patient communication
Establish a Speech Therapy treatment plan with patient and primary physician.
Assist in the implementation and development of patient care plans.
Prepare progress notes following visits for patients' charts.

What You Need:

Must have a Master's or Doctoral degree in speech-language pathology
Licensed to practice within the state of Florida
Member of the American Speech-Language and Hearing Association preferred.
CPR certification required (Online certification not accepted).
At least one year of institutional experience
