    Compensation:

    $31.86 Hourly

    What You Will Do

    • Assist with social work assessment and treatment planning
    • Provide ongoing social work services and advocacy for patients and families
    • Offer support in accessing entitlements and community resources
    • Provide psychoeducation and assistance with symptom management
    • Act as a liaison with community agencies and maintain communications as per VNS Health policy
    • Participate in related VNS Home Care activities and ensure professional growth

    Qualifications
    Education:
    Bachelor's Degree in Social Work or related human services field required

    Work Experience:
    Minimum one year of experience in mental health or social service setting required
